The problem

Multi-site and multi-provider programmes generate enormous amounts of data and remarkably little evidence. Each site records what it can, in the format it chose, on the cycle that suits it. What arrives centrally is a compliance artefact rather than a management instrument.
The same gap hides risk. Where records are inconsistent, the patterns that indicate fraud or error are indistinguishable from ordinary variation, and manual review only ever reaches a fraction of the caseload.
